Applications for Disease-Free Status
Owners of some types of livestock may apply to certify that their herds of free of, or test-negative for, some diseases.  This eases the requirements for export to other states or nations.  Cattle, bison, deer and elk herds may be certified to be free of brucellosis and tuberculosis.  Swine herds may be certified to be free of pseudorabies. Below is a list of forms you can download, complete and mail to the office of the Wisconsin State Veterinarian.  Please note that these forms are only for animals in Wisconsin.
